Abstract

Temporomandibular disorders (TMDs) occupy a unique space in dentistry. Although advances in diagnostic criteria, imaging, and pain science have improved our understanding of these conditions, many patients continue to present with symptoms that cannot be fully explained by objective findings alone. This disconnect creates one of the greatest challenges in TMD care: balancing scientific uncertainty with compassionate clinical practice. The absence of a definitive diagnosis should never be interpreted as the absence of disease or suffering. Rather, clinicians must recognize that uncertainty is an inherent feature of medicine and dentistry, particularly in chronic pain disorders. This commentary argues that intellectual humility—the willingness to acknowledge the limits of current knowledge while remaining committed to patient-centered care—is a fundamental clinical competency for practitioners treating TMD.

Healthcare traditionally rewards decisiveness. Patients understandably seek confident answers, and clinicians strive to provide them.

Yet confidence should not be mistaken for certainty.

One of the most important clinical skills in chronic pain management is the ability to communicate uncertainty without diminishing patient trust.

Statements such as:

"I do not yet know why you are experiencing these symptoms."

or

"Your imaging is reassuring, but it does not explain everything you are experiencing."

are not admissions of failure.

They are acknowledgments of the current limits of medical knowledge while affirming the patient's lived experience.

Patients rarely expect omniscience.

They expect honesty, curiosity, and commitment.

Implications for TMD Education

Dental education understandably emphasizes diagnostic classification, treatment planning, and procedural competency. These remain essential components of clinical training.

However, less attention is devoted to managing uncertainty.

Residents entering the field of TMD frequently encounter patients who have consulted multiple providers without satisfactory explanations. These interactions require more than technical knowledge.

They require communication skills.

They require emotional intelligence.

Most importantly, they require intellectual humility.

Teaching future clinicians to acknowledge uncertainty while maintaining patient confidence may prove just as important as teaching them how to interpret a magnetic resonance image or fabricate an occlusal appliance.

Redefining Success

In procedural dentistry, success is often defined by restoration, extraction, or surgical correction.

Chronic pain requires a broader definition.

Successful treatment may involve meaningful improvements in function, reductions in symptom severity, improved sleep, restoration of daily activities, or decreased fear associated with pain.

Complete symptom resolution remains an ideal outcome.

It should not be the only acceptable one.

Patients frequently judge clinical success not solely by whether pain disappears but by whether they feel heard, believed, and partnered throughout the diagnostic and therapeutic process.

Temporomandibular disorders remind us that dentistry is both a science and a profession grounded in human relationships. Scientific advances continue to improve our understanding of chronic orofacial pain, yet uncertainty remains an unavoidable aspect of clinical practice.

The measure of an exceptional TMD clinician is not the ability to explain every symptom immediately but the willingness to remain curious when explanations are incomplete, humble when knowledge is limited, and compassionate when patients are most vulnerable.

The absence of a diagnosis should never become the absence of care.
